IF-AT Forms

The IF-AT forms can currently be ordered in a length of 10, 25, or 50 questions with four or five answer choices.

The forms come in bundles of 125, and a total of four answer keys will be provided per order per variation. The answer keys are updated about every six to nine months

IF-AT forms are available for purchase in the US and over 20 selected non-US countries.If you would like to order IF-AT forms but you are not from one of the listed countries, please write ot us at ifat@cognalearn.com and we will be happy to assist.

  • Available in the US and outside of the US
  • Store forms away from the sun to prevent the curing of the silver foil
  • Curing will make scratching difficult
  • Good for use within 12 months of purchase
50Q4C blue background-1

25 / 50-Question Form

  • Available with 4 or 5 choices
  • 25 / 50-question 4-choice form is available to the US and non-US customers
  • 25 / 50-question 5-choice is available only to the non-US customers
  • Form can be used as a 50-question or a 25-question form
  • A perforation line between the 25th and 26th questions allows the form to be torn into two 25-question forms
  • Store forms away from the sun to prevent the curing of the silver foil
  • Curing will make scratching difficult
  • Good for use within 12 months of purchase
